Physical copies are available for in-person pick up at the front desk of the Illinois Department of Agriculture, John R. Block Building in Springfield. The John R. Block Building is located at 801 E Sangamon Ave, Springfield, IL 62702, which is open Monday - Friday from 8 am to 5 pm, excluding state holidays.
This report will also be available for pickup at the
