For ISBE PDHs you will be required to provide your IEIN.

We will use name to create your name badge, so please enter the name you prefer to be called.
If you cannot be reached at your school email during the summer, please provide an alternate email.
You can find your ISBE IEIN by logging into your ISBE ELIS account at https://apps.isbe.net/iwasnet/login.aspx
If you are paying for two participants below, please add the name and email address for the other person here. We will email them so they can register using a special "prepaid" code so they won't need to pay. (You'll have to work out the details between yourselves.)
If you bring your own lunch you will have access to a breakroom with a microwave if you need it. There are also fast food restaurants close by if you choose to purchase lunch.
Please print and sign the release and bring it with you to the first day of programming.

Refund Policy
Full refund available until two weeks prior to the first day of programming. If you are unable to attend, please request a refund by emailing amym5@illinois.edu by May 27, 2026. If the program is cancelled, fees will be refunded for all participants.
